Please submit together with the following details to 0866598853 / 011-4361941.

- CK 1 & CK2, or CM29 indicating Black shareholding (if any).
- Letter from bookkeeper / auditor confirming your annual turnover.
- Completed affidavits – must be signed by a commissioner of oath.
- Copy of ID Documents of all members/equity partners – please indicate race (B,W,I,C).
- Payment of R 855.00 inclusive of VAT – we will send through the invoice as soon as we receive the above documents.

Value Adding Suppliers – an entity whose Nett Profit Before Tax plus Total Labour Costs exceeds 25% of the value of its Total Revenue. Proof by means of an updated Income & Expenditure Statement issued by the auditor/bookkeeper will suffice.
Entities without bookkeepers / auditors need to supply financial statements or management accounts for the previous 12 months.
